AI Proof Gap Highlights Governance Shortfalls Despite Widespread AI Deployment

A significant divide is opening between executive ambition for artificial intelligence and the structured oversight required to ensure systems operate safely, deliver verifiable results, and withstand external review. Grant Thornton’s latest AI Impact Survey exposes this disconnect, revealing that most organizations deploy AI without the supporting frameworks needed for accountability and sustained effectiveness.

Conducted in early 2026 among nearly 1,000 senior US business leaders from diverse sectors, the survey shows that 78% of respondents do not have full confidence their organization could successfully complete an independent AI governance audit in 90 days. Additionally, half of operations leaders indicate a formal AI strategy or governance framework is required within the next six months to enhance overall performance.

“AI deployment continues to advance faster than the supporting structures designed to manage it,” noted Tom Puthiyamadam, Managing Partner of Advisory Services at Grant Thornton Advisors LLC. “This pattern has appeared with previous technologies, where protective measures are introduced only after problems emerge, often leading to substantial operational and organizational impacts.”

Governance Emerges as the Primary Barrier to AI Effectiveness

Survey data points to governance limitations — rather than the technology itself — as the core obstacle hindering AI performance. While 46% of leaders attribute underperformance to inadequate controls and compliance processes, just 11% believe the primary path to success lies in prioritizing risk and compliance efforts.

Scaling AI initiatives without first establishing proof of safety and reliability shifts the activity from innovation toward unmanaged exposure. Regulators are responding with heightened expectations. In the United States, the National Association of Insurance Commissioners (NAIC) has issued guidance outlining requirements for AI systems in insurance. In Europe, the EU AI Act, set for full enforcement of high-risk provisions in August 2026, along with related supervisory guidance, is driving insurers and other sectors toward more formalized oversight structures.

Limited Board-Level Ownership and Strategic Alignment

Despite broad board approval for major AI initiatives — reported by three-quarters of respondents — only 52% of organizations have established explicit governance expectations, and 54% incorporate AI-related risks and opportunities into regular board or committee discussions.

Traditional governance approaches often prove ill-suited for AI’s dynamic nature. Centralized approval processes can create delays without meaningfully lowering exposure. Experts recommend defining high-level policies and risk thresholds at the enterprise level while empowering trained teams at divisional or regional levels to conduct assessments calibrated to specific risk profiles.

Strategic clarity represents another key weakness. Although 51% of executives identify strategy as the leading factor influencing AI outcomes, only 22% of operations leaders confirm their organization has a fully developed and operational AI strategy.

“Teams are expanding AI use across additional pilots, applications, and business functions, yet many lack consistent ways to track results, implement feedback mechanisms, or pinpoint sources of value,” explained Sumeet Mahajan, Lead Partner for AI and Data in Advisory Services at Grant Thornton Advisors LLC. “Applying structured discipline — including defined performance targets, supporting infrastructure, and decisions to pause or stop underperforming efforts — becomes essential for progress.”

Managing Increased Autonomy in AI Systems

A growing number of organizations are advancing toward more autonomous AI capabilities, often referred to as agentic AI. Nearly three-quarters report piloting, expanding, or operating such systems. However, only one in five has developed and tested a dedicated response plan for potential failures.

While 95% of organizations prohibit fully autonomous AI from handling high-stakes decisions without human involvement, moderate-risk scenarios still carry notable exposure. Regulatory and compliance uncertainty ranks among the top concerns for 43% of leaders exploring agentic AI.

The greater vulnerability often lies not in system failures themselves but in the absence of tailored preparation. Standard incident response procedures frequently fall short when addressing AI-specific challenges such as model drift, inaccurate outputs, or unintended bias, which can complicate detection, explanation, and correction.

Organizations with Mature Governance Report Superior Results

The survey reveals a clear performance split between companies still primarily experimenting with AI and those that have embedded it more deeply into operations.

Entities with fully integrated AI programs are nearly four times more likely to report measurable revenue growth linked to AI (58% compared to 15% among those in the pilot stage). These same organizations also demonstrate higher confidence in their ability to pass independent governance audits.

“Organizations achieving stronger AI outcomes consistently prioritize governance as a foundational element,” Puthiyamadam concluded. “They focus on developing capabilities among their teams, tracking performance rigorously, and expanding only those applications that demonstrate clear results. Far from hindering progress, effective governance supports more reliable and enduring AI success.”
